Job summary

Vitae Health Systems seeks a Psychotherapist to deliver mental health services in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being and adaptation to medical life changes.

You will work with an interdisciplinary team, develop individualized plans, provide individual/group therapy, and document thoroughly to meet regulatory standards. Strong licensure and geriatric experience are preferred.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field
• Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
• Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
• Strong clinical assessment, diagnostic, and treatment planning skills
• Familiarity with electronic health records (EHR) systems preferred

Responsibilities

• Conduct psychosocial assessments for residents at admission and as needed
• Develop, implement, and monitor treatment plans
• Provide individual, group, and/or family therapy sessions
• Address depression, anxiety, grief, dementia, and adjustment to illness or disability
• Collaborate with clinical team, physicians, nursing, social workers, and rehab teams
• Participate in care plan meetings and interdisciplinary discussions
• Maintain accurate clinical documentation per regulations
• Provide crisis intervention and behavioral management when needed
• Educate staff and families on mental health and communication
• Ensure HIPAA compliance and ethical practice
