Job summary

Vitae Health Systems is seeking a licensed Psychotherapist to provide mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions to support overall well-being.

This full-time role offers $70,000-$90,000 base plus bonus opportunities, with options for base salary or commission. Requirements include a master’s degree and active license; 2 years of geriatric clinical experience preferred.

Qualifications

• Master's degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted state license (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling).

Responsibilities

• Conduct psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness.
• Collaborate with interdisciplinary team to coordinate care.
• Participate in care plan meetings and documentation.
• Provide crisis intervention and behavioral management when needed.
• Educate staff and families on mental health conditions.
