Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The role involves assessing, diagnosing, and treating emotional and behavioral conditions to support residents' well-being.

The position is part-time, non-exempt, with a compensation range of $35,000-$55,000 plus potential performance-based bonuses. On-site work in Illinois and opportunities for collaboration with a multidisciplinary team are offered.

Qualifications

• Masters degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in the respective state (LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of mental health conditions common in SNF populations (depression, anxiety, dementia, adjustment disorders).

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, attending physician, facility staff, social workers, and rehab teams to coordinate care.
• Participate in care plan meetings and interdisciplinary team discussions.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
• Educate staff and families on mental health conditions and effective communication strategies.
• Ensure compliance with HIPAA and ethical standards of practice.
