Carease Health is seeking a Mental Health/Care Companion to support older adults in skilled nursing facilities in the United States. You will provide emotional support, brief therapy, and mental health assessments, collaborating with psychiatric professionals to ensure comprehensive care.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsabilités

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
