Job summary

Carease Health in Pennsylvania is seeking a Mental Health/Care Companion to provide emotional support, brief therapy, and assessments for residents in skilled nursing facilities. You will visit facilities, assess needs, and document outcomes while coordinating with psychiatric providers.

The role emphasizes resident-centered care, regular supervision, and ongoing training, with a flexible daily schedule and a flat salary.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
