Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team in skilled nursing facilities. You will deliver emotional support, brief therapy, and mental health assessments to residents, promoting dignity and connection in their daily environment.

This role emphasizes direct impact on older adults, with regular supervision and collaboration with psychiatric providers, and opportunities for professional growth through ongoing training and clinical meetings.

Qualifications

• Degree in social work, psychology, counseling, or related mental health field.
• Experience in geriatric mental health and long-term care settings.
• Strong communication and documentation skills.

Responsibilities

• Visit designated facilities according to the schedule with your supervisor.
• Assess residents to identify emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions for immediate concerns.
• Document visit notes and maintain detailed records after each visit.
• Collaborate with psychiatric professionals, including monthly meeting with the psychiatric NP.
