Innovative Integrated Health, Inc. is seeking a physician to provide primary medical care to participants under the supervision of the Medical Director and in coordination with the Clinic Program Director.

The role includes assessment, care plan development with an interdisciplinary team, and direct patient care, with responsibilities for ongoing evaluation of the plan of care. The physician will work with nurse practitioners and physician assistants, perform physical exams, and coordinate

Qualifications

• Must be licensed to practice medicine in the state of California.
• Board-certified or eligible in internal medicine or family practice.
• Advanced certification in geriatrics preferred; DEA registration required.

Responsabilités

• Provide primary medical care to Innovative Integrated Health participants, including assessment and developing a plan of care with the IDT.
• Perform physical examinations, order labs/diagnostics, and adjust treatment plans as needed.
• Triage changes in participant status and coordinate with nursing staff.
• Maintain accurate documentation and participate in QAPI activities.
