Job summary

InnovAge Holding Corp. recruits a Physician to deliver medical care to enrolled geriatric participants across multiple settings, with most visits at the InnovAge Center clinic and some at homes, nursing facilities, and day centers.

Close coordination with hospital providers is essential, with mandatory on-site presence for 4 days of visits and 1 administrative day. The role requires an MD/DO, active state license and DEA, board certification or eligibility, and at least one year working with

Qualifications

• Medical degree required (MD/DO).
• Current active state medical license and DEA with full prescriptive authority.
• Board certification in primary care or board eligible with certification within 12 months.

Responsibilities

• Provide medical care including evaluation, diagnosis, and management of conditions.
• Document care in the medical record and support EMR enhancements.
• Rotational after-hours on-call coverage with RN triage.
• Participate in interdisciplinary care planning and care delivery.
• Contribute to quality improvement initiatives and new medical programs.
