Job summary

InnovAge is seeking a Physician to deliver comprehensive medical care for enrolled geriatric participants across our Day Center clinic, homes, nursing homes, and other centers. Most visits occur at the InnovAge Center clinic, with some specialized home or facility care.

The role requires an MD or DO, active state license and DEA with full prescriptive authority, and board certification in primary care or eligibility within 12 months.

Qualifications

• MD or DO required for practice.
• Board certification in primary care or eligible within 12 months.

Responsibilities

• Provide medical care to enrolled geriatric participants across multiple settings (clinic, homes, nursing facilities).
• Document care in medical records and support EMR enhancements.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Lead and motivate department, maintaining a positive culture.
