Job summary

Habitat Health seeks a Medical Assistant to support high-quality, participant-centered care within our PACE center. You will assist with intake, vital signs, phlebotomy, EKGs, immunizations, and collaboration with the Interdisciplinary Team to ensure smooth participant flow and accurate documentation.

You will work closely with providers to coordinate follow-up visits, maintain EMR records, and uphold safety and infection control protocols in a fast-paced environment.

Qualifications

  • High school diploma or equivalent; completion of an accredited Medical Assistant program.
  • Current Medical Assistant certification or credential as required by applicable state law and organizational requirements.
  • Current phlebotomy certification.
  • Demonstrated ability to perform routine clinical procedures within scope of practice, including participant intake, vital signs, screenings, specimen collection, and other clinical support activities.
  • Strong communication and collaboration skills, with the ability to effectively engage participants, caregivers, providers, and interdisciplinary team members to support coordinated, participant-centered care.
  • Strong organizational skills with the ability to manage multiple priorities, maintain attention to detail, and ensure timely follow-through in a fast-paced clinical environment.
  • Experience using Electronic Medical Records (EMRs) and the ability to quickly learn clinical documentation and other organizational systems.
  • Active CPR/BLS certification or ability to obtain certification within 30 days of hire.
  • Meets all applicable participant-facing health requirements, including required communicable disease screening, TB clearance, and immunizations.

Responsibilities

  • Supportsefficient participant flow by completing intake, rooming participants, obtaining vital signs and screenings, fulfilling provider orders, and preparing participants for examinations and treatments.
  • Performslaboratory testing, phlebotomy, EKGs, immunizations, injections, continuous glucose monitoring and other routine clinical procedures as directed by the healthcare provider and within scope of practice.
  • Partnerswith providers and the Interdisciplinary Team (IDT) to communicate relevant observations, participant needs, and changes in status that support coordinated, participant-centered care.
  • Coordinatesfollow-up visits, external appointments, annual and biannual assessments, and other clinical activities in collaboration with the care team to support continuity of care.
  • Serves as a first point of contact for participant questions and care-related needs, responding through phone calls, electronic inboxes, and other communication channels, and appropriately addressing or escalating requests to support timely, coordinated care.
  • Maintainsaccurate, timely, and complete participant intake and clinical documentation within the Electronic Medical Record (EMR) and other organizational systems.
  • Supportsclinical operations through referral tracking, medical record management and transmission, authorizations, and other administrative activities that facilitate timely participant care.
  • Maintainsclinic readiness by supporting routine clinic logs, exam room preparation and stocking, medical equipment readiness, and inventory management in partnership with center leadership.
  • Providesa welcoming, respectful, and participant-centered experience while supporting the unique needs of older adults and frail elderly participants.
  • Contributesto continuous improvement by proactively identifying opportunities to strengthen clinical workflows, participant experience, team collaboration, and quality of care.
  • Performs other related duties as assigned.
