Job summary

Rockville Center is seeking a skilled Licensed Practical Nurse (LPN) to join our geriatric-focused team in a Skilled Nursing Facility. The role requires NY licensure, independent assessment, and providing care in both Long Term Care and Skilled Unit, with duties including medication administration and vital signs.

Qualifications

• Current NY LPN license.
• Alternate weekends required.

Responsibilities

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs.
