Job summary

Rockville Center is seeking a talented Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The position requires NY licensure, the ability to work independently, and a passion for geriatric care in a fast-paced environment.

Responsibilities include medication administration, vital signs, resident assessments, and supervising CNAs, with emphasis on education for residents and families. Shifts are 7AM–3PM, with alternate weekends required.

Qualifications

• Current License-LPN.
• Alternate Weekends.

Responsibilities

• Provide care for residents in SNF and LTAC units.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
