Job summary

Rockville Center is seeking a licensed Practical Nurse (LPN) to join our skilled nursing facility team in New York. The role requires NY LPN licensure, the ability to assess and treat residents independently, and to perform functional therapeutic activities for geriatric residents.

Shifts are 7 AM–3 PM with alternating weekends. Responsibilities include medication administration (including IVs), vital signs, direct resident care, patient and family education, and collaboration with physicians,

Qualifications

• Current NY LPN license.
• Alternate weekends required.

Responsibilities

• Provide care for residents in the SNF and Long Term Care Unit.
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
