Job summary

Deer Oaks is seeking a Licensed Professional Clinical Counselor to join our integrated clinical team in long-term care settings. You will provide therapy and assessments to residents onsite, support care planning, and collaborate with interdisciplinary teams to improve outcomes.

The role offers Monday–Friday scheduling, autonomy, and robust administrative and clinical support to focus on patient well-being and quality of life.

Qualifications

• Active, unrestricted LPCC license.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide individual, family, and group therapy for LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical teams to coordinate care.
• Maintain timely documentation using the GNT and ensure regulatory compliance.
