Job summary

Deer Oaks is seeking a Licensed Professional Counselor to join our integrated clinical team onsite at long-term care facilities, providing therapy, assessments, care planning, and behavioral health support.

The role offers professional autonomy, a consistent Monday–Friday schedule, and robust administrative and clinical support to focus on high-quality geriatric care. You’ll collaborate with nurses, social services, rehabilitation and medical staff to coordinate care and improve outcomes.

Qualifications

• Active, unrestricted LPC license is required.
• Valid driver’s license and reliable transportation.
• Excellent written, verbal, and interpersonal communication.
• Ability to work independently and with an interdisciplinary team.
• Commitment to timely, high-quality documentation and care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, and treatment planning.
• Support residents with depression, anxiety, adjustment, grief, trauma, and other LTC concerns.
• Deliver behavioral management and brief, goal-oriented interventions.
• Collaborate with nursing, social services, rehabilitation, and medical staff.
• Maintain complete and timely documentation using our GNT system.
• Participate in care plan discussions and facility meetings as needed.
