Job summary

Deer Oaks Behavioral Health – Long-Term Care Clinical Services in the Midwest is seeking a Licensed Professional Counselor (LPC) to join our integrated clinical team onsite at local LTC facilities. You’ll deliver therapy, conduct assessments, and collaborate across disciplines with strong administrative support.

Monday–Friday schedule, with autonomy and ongoing clinical mentorship. Ideal candidates are LPCs with a passion for geriatric care, comfortable navigating long-term care environments,

Qualifications

• Active, unrestricted LPC license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfortable navigating LTC environments.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Coordinate with nursing, social services, rehabilitation, and medical providers to improve resident outcomes.
• Maintain complete and timely documentation using the GNT and HIPAA guidelines.
