Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join our integrated clinical team in long-term care facilities. You will provide therapy, assessments, and care planning on a Monday–Friday schedule with strong clinical support and administrative resources.

You will work onsite with older adults and medically complex residents, delivering evidence-based mental health care while coordinating with interdisciplinary staff and facility leadership.

Qualifications

• Active, unrestricted LPC license is required.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Reliable transportation with valid driver’s license.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and behavioral disturbances in LTC settings.
• Deliver behavioral management and brief, goal-oriented interventions aligned with geriatric care.
