Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking an LPC to join our integrated clinical team onsite in local LTC facilities. You’ll provide therapy, assessments, care planning, and behavioral health support with autonomy and strong administrative backing.

This role emphasizes collaboration with nurses, social services, rehab and medical staff, ensuring coordinated care while maintaining timely documentation and compliance.

Qualifications

• Active LPC license required.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfortable navigating LTC environments and providing therapy to older adults.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Coordinate with nursing, social services, rehabilitation, and medical staff for coordinated care.
• Maintain documentation using the Geriatric Notes Tool (GNT) and ensure compliance with HIPAA and professional guidelines.
