Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join our on-site team in long-term care facilities. You will provide therapy, assessments, and treatment planning within an interdisciplinary clinical environment, ensuring high-quality behavioral health services for older adults.

Qualifications

• Active, unrestricted LCPC license.
• Strong communication and collaboration with interdisciplinary teams.
• Excellent documentation and record-keeping.
• Ability to work on-site in LTC facilities with predictable schedule.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments and treatment planning.
• Support residents with depression, anxiety, and other LTC-related conditions.
• Collaborate with nursing, social services, and medical teams.
• Maintain timely documentation using GNT and comply with HIPAA.
