Job summary

Senior Care Therapy is seeking Licensed Professional Counselors to provide in-person psychotherapy to the geriatric population in SNF facilities across NJ, NY, PA, and MD. You will join a clinician-owned team that handles billing, credentialing, and pre-certifications, allowing you to focus on patient care.

Qualifications

• Active and unrestricted LPC license in the state applying for.
• Effective oral and written communication in English
• Proficiency with electronic health records (EHR) and documentation requirements.
• Ability to provide in-person services at facilities, up to 45 minutes per session.

Responsibilities

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioral health conditions including depression and anxiety.
• Develop and implement evidence-based treatment plans tailored to each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate clinical documentation in compliance with regulatory standards using the EHR.
• Monitor patient progress and adjust treatment plans as needed.
