Senior Care Therapy is seeking Licensed Professional Counselors (LPC) to provide in-person psychotherapy to the geriatric population in NJ facilities. The role supports patients in subacute and skilled nursing settings, with administrative tasks handled by SCT.

Qualifications

• Active and unrestricted LPC license in the state applying for.
• Effective oral and written English communication.
• Proficiency with electronic health records (EHR).
• Strong organizational and documentation skills with regulatory compliance.
• Ability to provide in-person services at facilities, up to 45 minutes.

Responsibilities

• Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral health conditions; develop evidence-based treatment plans.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate clinical documentation in accordance with regulatory standards using the EHR system.
• Monitor patient progress and adjust treatment plans as needed.
