Qualifications

• Active and unrestricted LPC license in the state of practice.
• Proficient English communication, both spoken and written.
• Experience with electronic health records (EHR).

Responsibilities

• Conduct individual psychotherapy sessions in subacute or skilled nursing facilities.
• Assess and treat emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans.
• Collaborate with facility staff and families.
• Maintain accurate clinical documentation in EHR.
• Monitor patient progress and adjust plans.
