Job summary

Total Life in Porterville-Bakersfield area is seeking nurse practitioners for an in-person contract role to lead longevity days at partner senior living communities. You will present educational content and then conduct one-on-one longevity visits with residents who sign up.

You will be trained by our Chief Medical Officer, with ongoing support, and will travel between sites. The schedule is on weekdays, with roughly 4–8 hours per site day as bookings grow.

Qualifications

• Active, unrestricted California RN license and NP certification from the California Board of Registered Nursing.
• An active NP Furnishing Number from the California Board of Registered Nursing.
• Current national NP board certification.
• Active DEA registration for California and registration with CURES, California's prescription drug monitoring program.
• Ability to practice under standardized procedures with a supervising physician, as required in California, or certification as a 104 NP under AB 890

Responsibilities

• Conduct longevity visits based on a comprehensive geriatric assessment: med review, cognition, mood, sleep, nutrition, mobility, and function.
• Evaluate eligible residents for weight management, including GLP-1 therapy when appropriate.
• Coordinate with primary care providers for follow-up care and referrals.
• Document encounters accurately to Medicare standards in the EHR.
• Travel to assigned senior living communities in the Porterville-Bakersfield area.
