Job summary
Senior Care Therapy, LLC is seeking Licensed Mental Health Counselors (LMHC) to provide in-person psychotherapy to geriatric residents in SNFs across NY, NJ, PA, and MD. This role emphasizes clinical care with full administrative support handled by SCT.

Qualifications
- Active LMHC license in the state applying for.
- Excellent verbal and written English communication.
- Experience with electronic health records (EHR).
Responsibilities
- Conduct individual and group psychotherapy sessions for residents in subacute or skilled nursing facilities.
- Assess, diagnose, and treat emotional and behavioral health conditions, including depression and anxiety.
- Develop and implement evidence-based treatment plans tailored to patient needs.
- Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
- Maintain accurate and timely clinical documentation in accordance with regulatory standards using the EHR.
- Monitor patient progress and adjust treatment plans as needed.
