Qualifications

• Active and unrestricted LMHC license in the state applying for.
• Excellent written and spoken English communication.
• Strong organizational and documentation skills with regulatory awareness.
• Ability to provide in-person services at facilities, up to 45 minutes.
• Proficiency with electronic health records (EHR) systems.

Responsibilities

• Conduct individual and group psychotherapy sessions in subacute or skilled nursing settings.
• Assess, diagnose, and treat emotional/behavioral conditions including depression and anxiety.
• Develop and implement evidence-based treatment plans tailored to patients.
• Collaborate with facility staff and families to support care goals.
• Maintain accurate clinical documentation per regulatory standards using the EHR.
• Monitor progress and adjust treatment plans as needed.
