Job summary

Senior Care Therapy, LLC is seeking Licensed Marriage and Family Therapists (LMFT) to provide in-person psychotherapy to residents in skilled nursing facilities across NJ, NY, PA, and MD. The role handles clinical care while SCT manages billing, credentialing, and pre-certifications, enabling clinicians to focus on high-quality patient care.

Qualifications

• Active and unrestricted LMFT license in state applying for.
• Proficiency in English (verbal and written).
• Ability to provide in-person services at facilities, up to 45 minutes.

Responsibilities

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans tailored to patient needs.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using our EMR.
• Monitor patient progress and adjust treatment plans as needed.
