Crane's Mill, a 5 star CCRC in West Caldwell, NJ, has a full-time opportunity available for a Licensed Social Worker. The position offers a competitive salary between $75k-$85k, robust benefits and paid time off.

We seek a BS/BA and MA in Social Work with NJ LSW, at least two years in long-term care. You will assess residents' psychosocial needs, support care planning, advocate for residents, and coordinate with families and staff in a faith-based environments.

Qualifications

• Education: BS/BA required; MA in Social Work required; LSW licensed in NJ.
• Minimum two years of experience as a Social Worker in a long-term care facility.
• Strong geriatric-specific assessment and care planning skills.
• Excellent verbal and written communication; ability to multi-task.

Responsibilities

• Identify and provide age-specific care per policy.
• Act as liaison between residents, families, responsible parties and staff.
• Advocate for residents while representing the facility’s interests.
• Maintain confidentiality and ensure HIPAA compliance.
• Complete timely social work assessments and discharge planning.
