Qualifications

• Geriatric social work competencies including the ability to identify and participate in the plan of care.

Responsibilities

• Supports the mission and values of Lutheran Social Ministries of New Jersey.
• Identify and provide age-specific care as needed.
• Act as liaison between residents, family, and staff.
• Serve as resident advocate while representing the facility’s interests.
• Maintain a safe environment and report unsafe conditions.
• Demonstrate effective communication and professionalism in all interactions.
• Ensure HIPAA compliance and proper documentation.
• Complete timely social work assessments and discharge plans.
• Coordinate care conferences with families and staff.
• Oversee lost and found initiatives and maintain documentation.
