Job summary

Innovive Health is seeking a Case Manager RN to partner with the Clinical Director and care team to coordinate skilled nursing services at patients' homes. You will assess, plan, implement, and document care in accordance with physician orders and agency policy.

The role requires an RN license, CPR certification, and reliable transportation, with 1–2 years of home health experience preferred. Travel within patients' homes may be required.

Qualifications

• Graduate of an accredited school of professional nursing.
• Licensed RN in the state and able to practice as an RN.
• BSN with 1 year of related experience or diploma/AD with 2 years experience.
• Must have CPR certification.
• Must possess reliable transportation and references.

Responsibilities

• Provides direct care and case management of assigned patients and develops plan of care.
• Maintains and updates schedules for all disciplines in the patient’s home.
• Assesses patient status and revises the plan of care as needed.
• Communicates plan changes with the team and physician.
• Oversees medications, refills, and patient education.
• Supervises Home Health Aide and LPN; ensures safety and quality of care.
