Job summary

Kintegra Health in Charlotte, NC seeks a Home Care Agency Nurse to develop and implement homecare services for frail elderly participants under the supervision of the STLC Home Care Agency Supervisor.

The role requires an Associate Degree in Nursing, an NC LPN license with a valid driver's license, and the ability to work in a team. On-call evenings and after hours are required as part of patient care.

Qualifications

• Associates Degree in Nursing required.
• At least 1 year as a LPN, preferably in home or community health.
• Licensed LPN in NC, current NC driver's license and vehicle.
• Must be able to work effectively in a team environment and treat geriatric patients.
• Meet CMS-approved competencies before working independently.
• Immunizations up to date before engaging in direct participant contact.

Responsibilities

• Assess home care needs of frail elderly population and develop plans of care.
• Perform in-home initial assessments, re-assessments, and supervisory visits every 90 days.
• Conduct in-home visits and nursing tasks as ordered by primary care provider.
• Maintain home care guidelines by updating policies and procedures.
• Ensure a safe, clean environment and coordinate with health care professionals as needed.
• Provide information to patients and health care team and address requests.
• Communicate changes in participant condition to supervisor and collaborate with interdisciplinary team.
• Assist in clinic setting when required by HCA supervisor.
• Be on call evenings/after hours when staff are in homes.
• Maintain vaccinations documentation in EMR and support mission of STLC Home Care Agency.
