Job summary

TALENT Software Services in San Diego is seeking a Home Care Coordinator to support the Home Care Coordination team. You will conduct home safety evaluations and coordinate meals, caregiver services, and DME while advocating for participants’ preferences in person-centered care.

You will collaborate with families and the interdisciplinary team, document progress in EMRs, monitor service delivery, and assist in adjusting care plans in compliance with regulatory guidelines.

Qualifications

• Must possess Home Health Aide and CNA certificates.
• CPR and First Aid certification required.
• Ability to coordinate and evaluate services within Home Health Aide scope.
• Must communicate clearly and function well in a team environment.

Responsibilities

• Coordinate IDT-approved meal delivery to client homes.
• Coordinate IDT-approved Durable Medical Equipment and incontinence supply delivery to client homes.
• Coordinate IDT-approved caregiver services for participants.
• Complete the Home Care Coordination care plan at enrollment and semi-annually thereafter.
• Conduct pre-enrollment, annual, semi-annual, and change of address home safety evaluations.
• Document and track actions, services, and service delivery status updates in designated logs and EMR.
• Communicate regularly with participants, families, and other healthcare providers to ensure coordinated care delivery.
• Advocate for participants' healthcare needs and preferences, ensuring care is person-centered.
