Job summary

Homeaides is seeking a Home Care Coordinator to organize and coordinate patient care in a home-based setting. You will work closely with clients, families, and caregivers to ensure safe, high-quality care in the clients’ homes.

You will manage assessments, care plans, scheduling, and compliance, while communicating effectively with providers and service coordinators to ensure continuity of care. The role requires compassion, reliability, and proactive problem solving.

Qualifications

• High school diploma or equivalent required.
• Demonstrated organizational and time-management skills.
• Excellent written and verbal communication abilities.

Responsibilities

• Conduct initial client assessments and identify individual care needs.
• Develop and maintain individualized care plans with clients, families, and coordinators.
• Monitor client needs and update care plans as appropriate.
• Maintain regular communication with clients and families to ensure satisfaction.
• Coordinate with service coordinators, healthcare providers, hospitals, and community resources.
• Assist with transitions of care and changes in client services.
• Schedule and assign caregivers based on client needs and availability.
• Monitor daily service delivery and respond to scheduling changes.
• Communicate schedule changes and updates to caregivers and clients.
• Monitor service authorizations and keep them current.
• Assist with recruiting, onboarding, and training caregivers.
• Provide ongoing guidance, coaching, and support to caregivers.
• Monitor caregiver performance and address concerns per policy.
• Ensure caregiver awareness of client care plans and requirements.
• Maintain a positive, professional work environment.
• Maintain accurate client and employee documentation.
• Ensure services follow care plans, policies, and regulations; protect HIPAA.
• Monitor service utilization and staffing documentation; participate in QA.
