Job summary

Habitat Health is seeking a dietitian to support older adults in a PACE-based program. You will perform nutrition assessments, develop care plans, and educate participants and caregivers as part of a multidisciplinary team.

Qualifications

• Master’s degree or equivalent in food science, nutrition, and/or dietetics as required by credentialing body.
• Nationally Certified as a Registered Dietitian Nutritionist.
• Ability to perform written assessments and document participant care.
• Experience with frail or elderly populations desired.

Responsibilities

• Develop and implement nutrition care plans for participants.
• Conduct nutritional assessments and reassessments.
• Provide nutrition education to participants and caregivers.
• Coordinate with PCP and care team for referrals and follow-ups.
• Monitor dietary intake and adjust plans as needed.
• Assist with meal planning and food access considerations.
