Job summary

BoldAge PACE seeks a Dietitian to assess nutritional status of participants and plan individualized interventions. You will work with the interdisciplinary team to deliver nutrition education, counseling, and therapy, coordinating 24-hour care and updating care plans as needed.

Engage with family and caregivers, incorporate cultural preferences, and support quality of life for older adults living at home. Training and growth opportunities provided.

Qualifications

• Bachelor’s degree from an accredited college with major studies in food, nutrition, or dietetics.
• Passing the Commission on Dietetic Registration (CDR) exam.
• Minimum of 1 year working with the elderly population required; training provided if needed.
• Minimum of two years clinical nutrition experience preferred.
• Serve Safe Certified preferred.
• NJ: Dietitian registered or eligible for registration.

Responsibilities

• Provide high quality nutritional care as a member of the IDT.
• Conduct initial comprehensive nutritional assessments upon enrollment and as needed.
• Collaborate with the care team to develop and revise care plans.
• Provide nutritional counseling to participants, caregivers, and families.
• Facilitate nutrition-related care plan interventions including meals and supplements.
• Monitor weight, diagnoses, drug-nutrient interactions, and hydration; report changes.
• Participate in IDT meetings and community outreach; maintain documentation.
