Job summary

Habitat Health is seeking a dynamic Dietitian to support elderly participants through comprehensive nutritional assessments, education, and care planning. You will work with a multidisciplinary team to implement nutrition interventions and monitor outcomes, ensuring person-centered care in alignment with Habitat PACE standards.

The role requires a Master’s degree in nutrition/dietetics and an active Registered Dietitian Nutritionist credential, with at least one year of elder care experience.

Qualifications

• Master's degree or equivalent in food science, nutrition, and/or dietetics as required by the credentialing body.
• Current medical terminology and ability to complete written assessments and document participant care.
• Nationally Certified as a Registered Dietitian Nutritionist.
• A minimum of one year experience working with the frail or elderly.

Responsibilities

• Develop and implement nutritional plans of care in collaboration with PCP and care team.
• Plan and execute follow-ups and reassessments for each participant.
• Assess food access and refer to the team for meals and groceries.
• Determine need for and order oral nutritional supplements.
• Provide nutrition education to participants, caregivers, and Habitat staff.
• Develop nutrition care plans for high-risk participants and provide medical nutrition therapy.
