Job summary

Cambridge Health Alliance (CHA) seeks a Registered Dietitian II to join the CHA PACE interdisciplinary team in Cambridge. You will perform comprehensive nutrition assessments and develop individualized plans to support health and independence for older adults.

Responsibilities include counseling, care-plan collaboration, monitoring outcomes, and occasional home visits. Requires MS, MA RD license, and geriatrics/nutrition experience; 32 hours/week, day shift, Tue–Fri, on site.

Qualifications

• Master’s of Science from an accredited institution.
• Massachusetts R.D. license (valid).
• 1 year geriatrics required; 1 year clinical or community nutrition preferred.
• 1–2 years ambulatory setting strongly preferred.
• Ability to transport between sites; computer literacy; Epic experience preferred.

Responsibilities

• Conduct nutritional assessments and develop tailored nutrition plans for PACE participants.
• Provide nutrition counseling to support healthy eating habits and chronic disease management.
• Collaborate with the interdisciplinary team to integrate nutritional care into the plan of care.
• Monitor and adjust nutrition plans based on outcomes and participant needs.
• Provide home-based visits as needed to support participant goals and safety.
