Job summary

Habitat Health in the United States seeks a dynamic Dietitian to join a multidisciplinary care team serving older adults enrolled in PACE. Qualifications

• Master's degree or equivalent in food science, nutrition, and/or dietetics.
• Current medical terminology and ability to complete written assessments.
• Nationally Certified as a Registered Dietitian Nutritionist.
• A minimum of one year experience working with the frail or elderly.

Responsibilities

• Develop and implement nutritional plans for participants with PCP and interdisciplinary team.
• Plan and execute follow ups and reassessments for each participant.
• Assess access to food and refer to meal planning needs.
• Determine need for and order oral nutritional supplements.
• Provide nutrition instruction to participants and caregivers.
• Develop and implement nutrition care plans for high-risk participants.
• Monitor food intake and communicate with the care team.
