Job summary

Habitat Health is seeking a Dietitian to join a multidisciplinary team delivering integrated nutrition care for older adults in San Leandro, CA. You will perform nutritional assessments, plan care with the primary care provider, and guide caregivers on dietary regimens during home visits and center visits.

Responsibilities include creating care plans, reassessments, and coordinating food access with the care team.

Qualifications

• Master’s degree or equivalent in food science, nutrition, and/or dietetics as required by the credentialing body.
• Nationally Certified as a Registered Dietitian Nutritionist.
• A minimum of one year experience working with the frail or elderly.
• Ability to thrive in a fast-paced, evolving environment with comfort navigating ambiguity.

Responsibilities

• Develop and implement nutritional plan of care for participants in collaboration with the PCP and interdisciplinary team.
• Plan and execute follow ups and reassessments for each participant.
• Assess participants’ access to food and refer to team for meal preparation and grocery needs.
• Provide instruction to participants and caregivers regarding dietary regimens and nutritional requirements.
• Develop and implement nutrition care plans for high-risk participants including medical nutrition therapy recommendations.
• Monitor food intake and dietary modifications and communicate with the care team.
