Zusammenfassung

PacificSource PACE LLC is seeking a Registered Nurse to provide direct clinic-based nursing care within a geriatrics-focused PACE program in Oregon. The role supports participant-centered care, assessment, treatment, vaccines, documentation, and cross-coverage for home visits as needed.

The ideal candidate has a current RN license, an associate’s degree, and at least 3 years of nursing or behavioral health experience, including work with frail older adults.

Qualifikationen

• Minimum of 3 years’ nursing or behavioral health experience.
• Experience with frail, older adults in state programs required.
• RN with current, unrestricted state license and an associate’s degree required.
• CPR certification required at time of hire or within three months.
• Knowledge of community resources and ability to use computerized data systems (EMR).

Aufgaben

• Develop, coordinate, and deliver individualized nursing care for PACE participants in the clinic.
• Perform clinical assessments, monitor condition changes, coordinate follow-ups, and support diagnostic tests and treatment plans.
• Provide direct clinic nursing care: vital signs, med administration, wound care, injections, immunizations, and specimen collection.
• Collaborate with physicians, nurse practitioners, and interdisciplinary team to ensure coordinated care.
• Educate participants and caregivers on treatments, self-care, and disease management.
• Support vaccine programs including planning, administration, and documentation.
• Maintain clinic readiness through inventory and supply management.
• Document all care activities accurately and timely in patient records.
• Participate in team meetings and cross-coverage for home nursing needs.
