PacificSource PACE LLC is seeking an experienced Registered Nurse to provide clinic-based nursing care within a geriatric PACE setting. You will perform assessments, vaccine administration, wound care, education, and care coordination with an interdisciplinary team to support participant health outcomes.

Responsibilities include vital signs, medication administration, vaccine planning, documentation, and cross-coverage for in-home nursing as needed.

Qualifikationen

• Active RN license in the state of Oregon.
• 3+ years nursing or care for older adults.
• CPR certification required at hire.
