Job summary

TALENT Software Services is seeking a Clinic Nurse in Modesto, CA to provide comprehensive nursing care under PCP supervision. Qualifications

• Minimum of one year nursing experience in a clinical setting with frail or elderly population.
• Ability to manage complex clinical situations and emergencies.
• Strong organizational and communication skills in senior care settings.

Responsibilities

• Perform and document ordered procedures and care in the clinic, including medication management and administration, as directed by PCP.
• Update RN Case Manager and clinic staff promptly of any changes in condition or status.
• Support smooth care transitions between settings (hospitals, SNFs, home, etc.).
• Dispense ordered medications to participants and reinforce education.
• Provide as-needed teaching to participants, family, and caregivers regarding disease, self-care, and prevention.
• Provide backup coverage to other nursing functions as assigned.
• Perform pharmacogenomic testing as directed by PCP.
• Support Medical Assistants performing procedures in the clinic.
• Process specialty pharmacy medication orders and escalate to MD as needed.
• Address participant medication issues and questions in clinic.
• Report medication incidents or errors at briefing and follow-up.
