Job summary

TEEMA Group seeks a Clinic RN to provide direct nursing care in a on-site outpatient clinic in Modesto, CA. You will manage medications, triage urgent concerns, and assist with wound care under the PCP and Clinic Supervisor guidance.

Ideal candidates have an ADN, an active RN license, and BLS, with at least 1 year in elder care or ambulatory care. Spanish language skills are a plus and strong multitasking is essential.

Qualifications

• ADN from an accredited nursing program and RN license in the practice state.
• BLS certification is required for this clinic role.
• Minimum of 1 year nursing experience in elder care or ambulatory settings.
• Reliable transportation for clinic-facing needs and mobility within care settings.

Responsibilities

• Provide direct clinical care and procedures per PCP direction, including med administration.
• Support triage, wound care, PGX testing, and in-clinic procedures with MAs and providers.
• Monitor patient condition and communicate changes to RN Case Manager and staff.
• Assist with patient care transitions between hospitals, SNFs, home, etc.
• Back up other nursing functions as assigned (Center Nurse, Meds MA, Case Manager).
