Job summary

TEEMA Group seeks a Clinic RN to provide direct nursing care in a on-site outpatient clinic in Modesto, CA. You will manage medications, triage urgent concerns, and assist with wound care under the PCP and Clinic Supervisor guidance.

Ideal candidates have an ADN, an active RN license, and BLS, with at least 1 year in elder care or ambulatory care. Spanish language skills are a plus and strong multitasking is essential.

Qualifications

Responsibilities

• Provide direct clinical care and procedures per PCP direction, including med administration.
• Support triage, wound care, PGX testing, and in-clinic procedures with MAs and providers.
• Monitor patient condition and communicate changes to RN Case Manager and staff.
• Assist with patient care transitions between hospitals, SNFs, home, etc.
• Back up other nursing functions as assigned (Center Nurse, Meds MA, Case Manager).

We are seeking a highly skilled Clinic Nurse to join our dynamic healthcare team! Operating under the direction of the Primary Care Provider (PCP) and supervision of the Clinic Supervisor, the Clinic Nurse provides direct nursing care, medication management, triage support, and wound care to older adults. This position plays a pivotal role in managing ongoing patient care, handling complex clinical situations, supporting care transitions across settings, and assisting Medical Assistants and providers with clinical procedures. What you will be doing: Direct Clinical Care & Procedures

• Perform and document ordered clinical procedures and care in the clinic, including medication management and administration, in accordance with scope of practice and PCP direction.
• Provide clinical support for triage, urgent clinical concerns, wound care, PGX testing, and assist Medical Assistants with in-clinic procedures.
• Remain alert to clinical observations and input from team members, patients, and caregivers; update the RN Case Manager and clinic staff promptly regarding any change in patient condition.
• Support smooth patient care transitions between healthcare settings (hospitals, skilled nursing facilities, home, etc.).
• Provide backup coverage as assigned for other nursing functions, including Center Nurse, Meds MA, and Case Manager roles.

Medication Management & Patient Education

• Dispense ordered medications to patients in the clinic, process specialty pharmacy medication orders, and address patient medication questions and concerns.
• Coordinate medication order changes with providers following same-day procedures and manage order updates from external providers.
• Perform pre-enrollment medication evaluations and report medication errors or incidents during morning briefings to drive continuous quality improvement.
• Instruct and provide ongoing education to patients, families, and caregivers regarding disease processes, self-care techniques, medication adherence, and prevention strategies.

Administrative & Clinic Operations

• Maintain clinical tracking logs for immunizations and screenings, including TB, Flu, Pneumovax, and Shingles.
• Monitor, inspect, and maintain the Emergency Cart monthly log, restocking supplies as needed.
• Manage medication storage systems, oversee the proper destruction of expired medications in accordance with policy, and maintain the Medication Destruction Log.
• Other duties as assigned

What you must have: Education, Licensure & Clearances

• Education: Associate’s Degree in Nursing (ADN) from an accredited nursing program.
• Licensure: Active, unencumbered Registered Nurse (RN) License in the state of practice.
• Certifications: Active Basic Life Support (BLS) Certification.
• Logistics & Mobility: Reliable transportation for clinic-facing operational needs.

Experience & Clinical Competencies

• Eldercare Focus: Minimum of one (1) year of nursing experience in a clinical setting caring for frail or elderly populations.
• Clinical Skills: Demonstrated expertise managing complex clinical situations, assessing changes in patient condition, prioritizing urgent care needs, and exercising sound clinical judgment in emergency situations.
• Organization & Independence: Highly self-directed and organized to perform multi-task workflows for multiple patients with minimal supervision in a fast-paced setting.
• Interpersonal & Cultural Skills: Outstanding clinical, organizational, and verbal communication skills; experience working respectfully with individuals from diverse cultural backgrounds.

Desired Qualifications

• Acute Care Background: Prior nursing experience in acute care settings (e.g., ICU, Telemetry, or Medical-Surgical units) is highly valued.
• Outpatient & Primary Care Experience: Background in primary care, ambulatory care, geriatrics, community health, or care coordination.
• Language Skills: Bilingual verbal and written fluency in English and Spanish is a plus.
