Job summary

Requirements include a Master's in Social Work; LCSW or LICSW preferred. 2–3 years of clinical experience in a medical setting.

Qualifications

• Master's Degree in Social Work required.
• Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) preferred.
• 2–3 years of clinical experience in a medical setting.
• Ability to formulate biopsychosocial assessments and treatment plans.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
