Job summary

Bethesda Health Group in Missouri is seeking an experienced RN to coordinate and direct hospice services for assigned patients, ensuring high-quality care and timely communication with families and physicians.

You will perform comprehensive assessments, develop and update the plan of care, document in the EMR, and participate in on-call coverage, case conferences, and QA/QI activities while maintaining accurate visit schedules and regulatory compliance.

Qualifications

• Graduate of accredited school of nursing.
• Current Missouri RN license in good standing.
• Minimum two years of varied work experience as a professional nurse, preferably home health/hospice.
• Strong written and oral communication and documentation skills.
• Ability to lift/move patients with assistance when needed.

Responsibilities

• Organizes, coordinates and directs ongoing hospice services for assigned patients.
• Performs initial and follow-up comprehensive assessments and drug reconciliation.
• Develops and updates the plan of care with patient, family and physicians.
• Documents in EMR and maintains accurate visit schedule and records.
