You will participate in multidisciplinary rounds, educate patients and families, and help arrange post-acute services to ensure timely discharge planning.

• Proficient in electronic medical records and Outlook email.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness impact and resources.
• Assesses readmitted patients and analyzes reasons for readmission.
• Organizes and facilitates care conferences with the team.
• Documents discharge plans and communicates with payors for post-acute care.
