Qualifications

• RN required with nursing degree and licensure.
• Experience in discharge planning or care management preferred.
• Knowledge of CMS Conditions of Participation for Discharge Planning.

Responsibilities

• Participates in rounds to review patient status and discharge plans.
• Educates patients and families on illness impact and resources.
• Assesses readmitted patients' reasons for readmission.
• Organizes and facilitates patient/family care conferences.
• Documents discharge planning and communicates with payors for post-acute care.
• Assesses wholistically for discharge planning needs across settings.
• Reviews medical records and incorporates factors into transition of care plan.
• Develops discharge plans with contingency plans for timely care.
• Uses technology to coordinate with post-acute care services.
