Job summary
AdventHealth in Florida is seeking an experienced Registered Nurse for patient discharge planning and care coordination. You will participate in multidisciplinary rounds, assess patient needs, educate families, and mobilize community resources to support timely discharge and ongoing care.
The role requires an active RN license, 2+ years of hospital nursing experience, and knowledge of CMS discharge planning requirements.
Qualifications
- RN licensure required; active license in Florida preferred
- 2+ medical/hospital nursing experience required
- Care management/utilization management experience preferred, CMS familiarity a plus
Responsibilities
- Participates in multidisciplinary rounds to review patient status, progression, and discharge plans.
- Communicates with patients and families about emotional, social, and financial impacts of illness; mobilizes community resources.
- Documents discharge planning evaluation, plan, barriers, and progress in medical records.
- Coordinates post-acute care arrangements and communicates with payors for authorization as needed.
- Ensures discharge planning considers functional status, support systems, and home care needs.
