Job summary

HarmonyCares is seeking a Value-based Physician Leader in Kalamazoo to oversee clinical pods, supervise APPs and collaborate with physician teams. You will drive quality care, documentation accuracy, and patient safety for high‑risk geriatric populations, with a Mon-Fri schedule and no weekends.

You will partner with market leadership to align clinical goals with organizational strategies. The role emphasizes leadership, performance metrics, care coordination, and education across the pod, with

Qualifications

• Bachelor’s Degree required.
• Doctor of Medicine or Osteopathy required.
• Valid driver’s license with clean record is preferred.
• Active CPR Certification required.
• Unrestricted DEA required.
• Residency Program completed.
• Unrestricted Medical License required.
• Board certification required for hires on or after Jan 1, 2026.

Responsibilities

• Act as on-site supervising Physician for assigned APPs and collaborating physicians.
• Ensure collaborative practice agreements comply with state regulations.
• Lead and participate in high-risk patient huddles and ensure care plans include all team members.
• Develop APP clinical development and promote evidence-based care and professionalism.
• Lead clinical education on accurate diagnosis, documentation and chronic disease management.
• Improve EMR efficiency and assist pod providers with patient care questions.
