Job summary

HarmonyCares is seeking a Medical Pod Leader to oversee the clinical performance of pod teams, ensuring accurate diagnosis, documentation, patient safety, and provider productivity. The role requires supervising APPs and partnering with the Market Manager to maintain high-quality care.

You will lead clinical education, participate in high-risk care planning, and drive improvements across chronic conditions while maintaining strong patient relationships and efficient care delivery.

Qualifications

• MD/DO licensed to practice in the United States.
• Board certified; active in good standing or must obtain certification as required by policy.
• CPR, DEA and medical license must be current.
• Experience collaborating with APPs and leading clinical teams.

Responsibilities

• Act as on-site supervising physician for APPs and POD physicians.
• Participate in rides along with APPs and provide complex patient visits as needed.
• Collaborate with pod team members (nurses, social workers, care coordinators).
• Lead high‑risk huddles and ensure care plans are documented across the team.
• Review quality metrics and drive improvements in care delivery.
